In my work as a clinician, I've spent nearly two decades watching the same pattern repeat itself: the treatments that earn approval through randomised controlled trials, our gold standard for evidence, are often tested on populations that don't look like the patients sitting in my clinic. The gap between what clinical trials demonstrate and what I observe in clinical practice isn't merely theoretical—it's reflected in patients sitting in our clinics—and this is precisely where real-world evidence becomes not just valuable but essential.

Something has shifted in the room.

The recent GetReal Conference 2026 in Utrecht, brought together regulators, health technology assessment (HTA) bodies, payers, academia, and life science partners, and the conversation about real-world evidence (RWE) felt different. The question is no longer whether RWE belongs in the evidence ecosystem. It is now about when to use it, how to use it well, and which type of evidence is the right fit for the question at hand. That shift, from skepticism to strategic integration, was palpable throughout every session.

Here is what we heard, and what it means for the future of evidence generation in oncology.

Die Generierung von qualitativ hochwertiger Real-World Evidence (RWE) in der Onkologie ist eng verknüpft mit der präzisen Erfassung des Gesamtüberlebens (Overall Survival). Verlässliche Mortalitätsdaten sind dabei fundamental für vergleichende Wirksamkeitsstudien sowie für die Vorlage bei Zulassungsbehörden. Allerdings stehen elektronische Patientenakten (EHR) in Deutschland vor der Herausforderung, dass die Erfassung von Todesfällen unvollständig sein kann, etwa wenn Patienten außerhalb des Behandlungsumfeldes versterben oder die Nachverfolgung anderweitig unterbrochen wird.
